Submit a Firefighter for Inclusion on the National Memorial

The National Fallen Firefighters Foundation was created by Congress in 1992 to honor and recognize firefighters who die in the line of duty each year. To meet this mandate, the Foundation reviews the circumstances of each firefighter fatality reported to determine whether the death also meets the
criteria for inclusion on the National Memorial in Emmitsburg, Maryland.

The Roll of Honor lists the names of all firefighters honored at the National Fallen Firefighters Memorial since 1981. Please use this form to submit information about firefighters who have not yet been honored on the National Memorial. The NFFF Research Team will review each submission to determine eligibility for inclusion.

Additional Information Related to Cause of Death

Additional documentation is requested for cardiovascular, cancer, COVID‐19, 9/11‐related illness, and suicide‐related deaths:

Cardiovascular
For deaths resulting from a heart attack or stroke, documentation must be provided showing the firefighter’s participation in emergency response or training activities within 24 hours before the onset of the cardiovascular event. Examples of documentation include a copy of the Public Safety Officers’ Benefits determination, department incident or run reports, newspaper articles, notarized witness statements, hospital records, physician reports, and disability records. Please submit any relevant documentation.

Cancer
Deaths directly resulting from cancer that occurred on or after January 1, 2018, that are defined as meeting the criteria of the decedent’s home state occupational exposure presumption laws are eligible for inclusion on the National Memorial. Please submit the line‐of‐duty death documentation from the decedent’s state.

COVID‐19
A copy of the determination documentation from the Public Safety Officers’ Benefits Program is requested for firefighters who have died due to COVID‐19. Please submit a copy of the PSOB determination.

Suicide
A copy of the determination documentation from the Public Safety Officers’ Benefits Program is requested for firefighters who have died due to suicide on or after January 1, 2019. Please submit a copy of the PSOB determination.

9/11‐Related Illness
A copy of the determination documentation from the Public Safety Officers’ Benefits Program or September 11th Victims Compensation Fund is requested for firefighters who have died due to 9/11‐Related Illness. Please submit a copy of this documentation.
